HopeCare Clinic-Family Planning

The HopeCare Clinic is open 5 days a week and appointments are accepted. We are open to the public and no referral is required. Adolescents, men, and women of reproductive age welcome! Services are provided at no or very low cost; Medicaid and private insurance accepted.

The Clinic is staffed by a nurse practitioner who performs routine testing and examinations for men, women and adolescents. English & Spanish spoken.

  • Medical Exams: annual exam, pap smear, breast exam
  • Birth control available
  • Pregnancy testing and counseling
  • Emergency contraception
  • Testing, treatment, and counseling for sexually transmitted infections
  • Urinary tract and vaginal infections testing and treatment

Telehealth Services Now Available! 

  •  See one of our providers from the comfort of your home
  •  Can be used to prescribe several forms of birth control including pills, patch and vaginal ring
  • Can be used if patients have limitations such as issues with childcare or transportation
  • Use for follow up on test results
  • Can be used for new or established patients
  • Patients can use a phone or a computer with audio and video capabilities and to have zoom downloaded
  • No extra cost to the patient
  • English & Spanish spoken.

Healthy Families

Healthy Families is a voluntary and confidential home-based service to support new and expectant parents. Because babies begin learning at birth, Healthy Families believes parents are their child’s first and best teachers. Parenting is a very demanding job and all parents have worries, questions and stress.

The program provides families with information about services available in the community, information about the health, safety and care of newborns, and access to the latest information on brain development. Families receive regular visits from a trained Family Support Specialist who can provide support, developmental assessments for their child, and an opportunity to set and accomplish family goals.

Healthy Families believes:

  • Parents are most knowledgeable about what services their family needs

Healthy Families offers:

  • The latest information on infant and child brain development
  • Age-appropriate developmental screenings

Employment Services

The Employment Services program assists individuals in selecting, obtaining, and/or maintaining appropriate employment in competitive settings. Employment Consultants will assist clients to develop a Plan for Employment, build a resumé, perform a job search, and assist with the application and interview process. Once a job is secured, Employment Consultants can work with clients to help with job stability.

Office hours are 8:00am – 4:00pm, but on-the-job assistance will be coordinated with your work schedule once employment is secured.

Community Habilitation

The Community Habilitation program provides purposeful, goal-oriented activities for individuals with mild to moderate intellectual or physical disabilities. It focuses on community integration but also incorporates activities to increase Activities of Daily Living (ADL) skills.

Community Habilitation is designed to assist clients with reaching their highest level of independence when it comes to personal, social, and daily living skills, and it works to promote community inclusion. The program incorporates individual services and small groups of 2-8 clients. Services are based on an individualized plan of care.

The program operates Monday through Friday, 8:00am – 4:00pm, starting at New Hope Center in Jeffersonville, at Gardner Place in Scottsburg, or at an individual’s home before venturing out into the community.

Individualized outings can also be arranged for evening or weekend hours based on client need.

This service is offered in Clark, Floyd, Jefferson and Scott counties.

Behavior Management Services

The Behavior Management Services program supports individuals to use positive strategies to decrease the occurrence of unwanted behaviors. Utilizing these services can enhance an individual’s quality of life by increasing their independence and meaningful participation in the community.

New Hope’s Behavior Specialists partner with clients, staff, and family members to provide hands-on training to promote success and decrease the number of challenging or dangerous behaviors. Behavior Management Services are funded through the CIH, FSW Medicaid Waivers.

Behavior Management Consultant

Janis has over 20 years of experience working with individuals who have developmental disabilities, first in a classroom setting and for the last 10 years as a Behavior Consultant. Janis earned her Bachelor’s Degree in Psychology from Indiana University Southeast and her Master’s Degree in Behavior Analysis from Spalding University.

Ja’Dora Parks, MA
Behavior Management Consultant

Ja’Dora has a Master of Arts degree in Professional Counseling from the University of the Cumberlands and a BA in Mental Health Counseling from Lindsay Wilson College. She has previous experience as a Behavior Clinician, Clinical Therapist, and Clinical Director.

Lisandra Sepulveda, MA
Behavior Management Consultant

Lisandra holds a Master of Arts degree from the University of Nevada, Reno and earned BA degrees in Psychology and Criminology. She has over 20 years of experience providing one-on-one behavioral therapy, completing functional behavior assessments, designing programs, conducting training workshops, and developing behavior intervention plans. She has worked with both children and adults with various intellectual disabilities in home, school, and residential care settings.

This program is currently offered in Clark, Floyd and Scott counties.
